The Phylrich Rewards program is being discontinued. Only claims received by 1/31/2012 will be considered.

To have your claims qualify, please fax copies of your invoices along with the claim form by January 31, 2012 to:

Qualifying products include Lavatory Faucets, Shower Valves and Tub Sets.

To qualify for rewards,

(1) sell a K-Series lavatory faucet and receive $15.00,

(2) sell a Tub Set and receive $15.00,

(3) sell a shower valve and receive $15.00.

Rewards are $5.00 for every select D-Series faucet sold.
